Thank you for signing up for the 4th Annual Khayamandi Golf Tournament.

We are thrilled to have you participating! This form will collect your information and the information of those you want to play with for the tournament.

Your payment can be separate or together. Following your submission, you will see a message with link to pay for your registration and/or sponsorship through our Secure Give App. You can always go to the Khayamandi Foundation and click donate to make your payments/donations.

If you are unsure of the names of the players on your team, we will send the form out to you again closer to the event.
